Choosing SAC Health as Your Primary Provider

Our mission to serve patients with special financial circumstances ensures that no one requiring medical services is turned away, regardless of their current insurance or employment status. In order to receive low and no cost services, we qualify each patient for the programs for which they are eligible for. This process involves filling out paperwork and takes time. Please arrive at least 15 minutes early for your appointment for this process.

If you are not yet a SAC Health patient, below are some recommendations on how to choose or switch your primary care provider to SAC Health.

Before you make an appointment, call your insurance provider to choose or change to SAC Health as your primary care provider.

Choosing or switching to a new primary care provider must be done by the first day of each month

Your insurance provider typically provides a list of physicians that are affiliated with SAC Health

Visit our locations page and choose the clinic that is closest or most convenient to you.

To make an appointment for all SAC Health services, please call (909) 382-7100. You may also log into your MyChart portal and schedule an appointment online when available.

Effective September 10, 2023, our appointment center hours of operations will be extended to 7:00am - 7:00pm Monday - Friday.

Appointments

Once you have scheduled an appointment, we ask that you bring the following items:

Photo Identification

Proof of Income (if no insurance)

Basic patient information

Insurance Card (if any)

Immunization Record (if pediatric)

List of current medications (if any)

Please come to your scheduled appointment 15 minutes early. We will do all we can to get you in on time, but please expect that there may be delays. We appreciate your patience.

When calling for your appointment, please have patient name, date of birth, and insurance coverage information.

SAC Health has contracts with many insurance companies and accepts most state and federal funding including IEHP, Medi-Cal and Medicare.

We accept Visa, MasterCard, and cash. We also accept most insurances. If you have insurance, please bring your insurance cards with you. If you don’t have insurance, we can still help you. 

Depending on your needs and eligibility, we can help you access no cost programs. If you don’t qualify, we also offer a sliding fee scale and Blessing Fund. Based on your family income and number of family members in your household, you may be eligible to pay only a nominal fee.

If you have a change in the size of your family or how much money your family makes, please notify the front desk, as this may change the program for which you are eligible.
